Elective Surgery Policy

Prior to your surgery, our staff will contact your insurance company to obtain insurance benefit information: dates of eligibility, benefits, co-payments, co-insurance and deductible amounts.  Every effort is made to obtain the most current information your insurance carrier can provide. High Pointe Surgery Center has no way to guarantee the accuracy of the information given to us by your insurance carrier.  Your surgeon, anesthesia, and pathology services are billed separately from High Pointe Surgery Center’s facility fee.

Our staff will contact you to discuss the anticipated patient obligation for the procedure and request a down payment for the patient deductible or coinsurance, or discuss payment plan options.  If your deductible has been satisfied prior to your surgery, a payment will not be required. If you have specific questions about your insurance plan, please contact your insurance carrier directly.

If applicable, a refund will be mailed to you within 30 days after High Pointe Surgery Center receives notification from your insurance carrier and your claim is finalized. Any amounts owed above the deposited amount will be billed to you. Payment in full is requested within 90 days of your date-of-service.

This payment policy excludes cosmetic procedures not medically necessary.
